Steps for the Accommodation Process
- Submit an intake form via the website. Intake Form
- Students receive an automated response with a Functional Assessment Form (FAF) attached in that email as a PDF. They can either get the FAF filled by a medical professional or submit any supporting medical documentation they may have (for example: psych assessment IPP/IEP from high school, doctor’s note (need the diagnosis).
- Please note that documentation is not necessary for the initial appointment.
- A file will be created under the student's name with the intake form.
- Accessibility services will reach out to the student with an appointment booking link.
- These appointments can be done in person, by phone, or through video chat (MS Teams). They are roughly an hour long.
- The student will meet with their advisor and will go over their intake form and documentation if available. At that point, they will generate an Accommodation Memo that states what accommodations have been decided on. It can include testing accommodations and class accommodations. This memo also states instructor and student expectations.
- The student brings the memo to their instructors, and they talk about how they will implement the accommodations in the classroom. If the instructor says they aren’t able to accommodate extra time in class, then the student may choose to write in testing services.
