The first step to meet with an Accessibility Advisor is to complete the Accessibility Services Intake Form via this link (Please be sure to digitally sign the “consent” portion of the intake form before submitting)
Medical documentation is not required for this initial appointment. If you do have medical documentation you can fax it to us (403-210-4557), bring it to the office in person, or email it to us.
This email is monitored exclusively by accessibility services staff, however, if you're concerned about privacy I have attached instructions on how to password-protect a PDF. Please use the password "rose2020" if you decide to password-protect your document.
Once we receive the completed intake form, you will receive further notification on the necessary next steps in meeting with one of our Accessibility Advisors.
